Poor rear shifting when on large chain ring

Using Campy Chorus 11-speed mechanical system.
I have had a long-standing issue getting the rear shifting right when the front is on the large chain ring.
We have tried literllay everything inc;luding: adjusting cable tension, replacing cable, swapping out rear derailleur, and checking hanger alignment. Nothing seems to work.
It's not everry shift but most in the middle of the cassette are not smooth and regularly hang up or over shift. But only when chain is on the large ring.

Any thougths would be appreicated.

Tough to diagnose when you don't list any information beyond groupset.

Did you check chain length as well?
