I admit I had to Google as I didn't know who Jan-Willem van Schip was but very interesting screenshots. Despite the focus here being on the narrow bars and extreme position (which I agree makes my core and triceps burn just looking at it), the bike itself is quite intersting. Look at the huge stack of spacers on the front, what looks to be an stem with a positive rise, miles of exposed cable (albit on a very large size bike), and a very odd looking seat tube. I've never seen anything like it.

Yes, those bars are very narrow at the hoods, but with the levers canted inwards, I see that his forearms cant outwards such that the spacing between his elbows appears to be nearly the same width as his shoulders.

love that crazy attitude, i wish more pros would tweak with their setups like that. of course cables or beard cost him few watts but in this case i bet it's the position that makes him faster. sometimes it's more important how you feel about being set up on a bike than what the numbers from the armchair wind tunnel testing suggest

He also appears to have a reversed or foward tilted seatpost. Maybe it's a Triathlon seatpost option for that frame?
Approximating here, but from the images it seems that the hip flexors of the rider in question are on the same level as the eyes of the rider directly behind him - so at least 30 cm (12 inches) higher.
